Santa Casa de Aparecida allows you to request copies of medical documents, such as medical records, exams and hospital care reports. These documents may be requested for purposes such as continuing medical treatment, legal proceedings, expert reports or other duly justified reasons.

How to Request

You can make the request in two ways:

By sending an email to same@scma.org.br with your requests and needs.

In person, at the reception desk of Santa Casa de Aparecida.

Be sure to include the complete information of the patient and the applicant, in addition to attaching copies of the ID or driver's license of both (if the request is made by third parties). Detail which documents you need (e.g.: exams, hospitalization) and the reason for the request.

Delivery Deadlines

After the request, the copies will be delivered within the following deadlines:

For services provided up to 30 days ago: 10 business days.

For appointments made between 30 days and 6 months ago: 15 business days.

For appointments made between 6 months and 1 year ago: 20 business days.

For appointments made more than 1 year ago: 30 business days.

Important

Only the patient or people with a proven legal relationship may request copies (e.g., spouse, children, parents or attorneys).

A fee of R$ 0.50 will be charged per printed page. If you prefer, we encourage you to send the copies by email, which will reduce costs.

Opening Hours

Copies can be picked up from Monday to Friday, from 10 am to 11 am or from 2 pm to 3 pm.
